MDS Coordinator (RN)

We are looking for an experienced MDS Coordinator (RN) to support two skilled nursing facilities in the region. This role will be based primarily out of one location, with travel to a second location as needed.

Responsibilities include:

  • MDS completion and submission

  • Assessment scheduling

  • Care planning

  • PDPM support

  • Coordination with the interdisciplinary team

Requirements:

  • Strong SNF/LTC MDS experience

  • Active RN license

  • Ability to travel between locations as needed

We offer health insurance, dental, vision, PTO, and a 401(k) with match. Mileage reimbursement available.

Pay: $45-$50/hour

Salary : $45 - $50
