Are you a detail-oriented nurse with a passion for both clinical accuracy and team education? Northern Lights Community is seeking a Full-Time MDS Coordinator to ensure top-quality care for our residents in assisted living and memory care, while supporting the growth and development of our dedicated care team.
In this dual role, you’ll play a critical part in coordinating and managing individualized resident assessments in compliance with all state and federal guidelines. You’ll also help shape a culture of continuous learning by identifying and fulfilling the educational needs of our staff.

What You’ll Do (AKA: Your Superpowers)
Coordinate Accurate & Timely Resident Assessments:

Manage and track MDS assessments (initial, quarterly, annual, and significant change) in alignment with OBRA regulations.

Schedule and communicate RAI (Resident Assessment Instrument) requirements with the interdisciplinary team.

Ensure all assessments and related documentation are complete and compliant to support care planning and reimbursement accuracy.

Submit MDS/RUGS data electronically to meet Medicare, Medicaid, and OBRS requirements.
Ensure Documentation Excellence & Clinical Accuracy:

Review nursing documentation regularly to ensure clarity, consistency, and clinical relevance.

Guide teams in accurate ADL documentation to support strong clinical outcomes and appropriate reimbursement.

Maintain an accurate and current Roster Matrix and monitor key comorbidities and NTAs for optimal coding.
Educate, Empower, and Elevate Your Team:

Provide education to staff on MDS processes, ADL scoring, and documentation best practices.

Support ongoing clinical training, mentoring, and professional development for team members.

Collaborate with the Director of Nursing to address additional educational needs and enhance staff competency.

What You Bring to the Table (Besides Clinical Knowledge & Heart)
Licensure & Experience:

Active RN license (Wisconsin) required.

Previous experience with MDS coordination and knowledge of RAI/MDS 3.0 processes preferred.

Strong understanding of Medicare/Medicaid guidelines, RUGs/CMI scoring, and care planning required.
