Job Title: LCSW/CSW
Starting hourly pay: $62,400 and up Depending on Experience
Location: Davis and South Salt Lake City
Schedule: Full-time, 40 hours Monday- Friday

General Description:
Licensed Clinical Social Worker or Clinical Social Worker, to provide counseling and case management services that address the social and emotional needs of our patients. The clinician must possess excellent clinical skills and will be responsible for providing short-term mental health counseling, group therapy, and crisis management using evidence-based treatment models with adults, children, and families. Supports the provision of comprehensive care by counseling and making referrals to appropriate agencies or providers. Works closely with the primary health care team to provide behavioral health consultation and intervention.
Responsibilities:
- Provides assessments that are consistent with the age group and population involved
- Understands and identifies mental health issues that are lifecycle-related
- Utilizes therapeutic techniques that are age-appropriate
- Provides treatment within the scope of practice and training
- Able to provide individual, group, and or family counseling sessions
- Provides psycho-educational services
- Assists patients in seeking charity care providers and community resources by building referral sources.
- Provides counsel and support to patients suffering from drug or alcohol dependency, and educates families on the best ways to help in the recovery process.
- Designs a program of treatment and rehabilitation tailored to the patient's situation based on knowledge of alcohol and drug dependency and counseling and treatment methods.
- Seeks appropriate consultation with mental health providers/supervisor regarding patient care.
- Consults with medical providers regarding patient's mental health issues and progress in psychotherapy.
- Complete and accurate documentation of care.
- Follows agency protocol for keeping and storing patient information.
- Appropriate documentation of critical incidents/patient concerns.
- Completes in-house encounters within 72 hours.
- Collaborates and interacts professionally with community agencies.
- Adheres to external regulations including CLIA, OSHA, and BPHC
- Participates in continuing education as necessary for licensure.
- Participates in the supervision and training of students as appropriate.
- Participates in annual peer reviews
- Accepts walk-in patients/make appropriate accommodations.
- Participates in the supervision and training of students as appropriate.
Minimum Qualifications:
- Master's Degree from an accredited Social Work Program.
- Two years of experience working with individuals with drug or alcohol abuse disorders under close supervision is preferred.
- License required or eligible and working towards licensed clinical social worker required, current Utah license preferred.
Other:
Ability to communicate in English and Spanish preferred to meet the mission of Midtown. Experience with underserved populations preferred.
Salary : $24 - $62,400
