POSITION SUMMARY

The Manager of Mental Health & Aftercare Services leads the development, coordination, and delivery of mental health support programming through coordination and post-exit aftercare services for individuals served by The Midnight Mission. This role serves as the organization’s primary liaison with external mental health agencies, county behavioral health systems, substance use treatment providers, and community-based clinical partners.

The Manager ensures that TMM’s programming is grounded in trauma-informed, client-centered approaches and that individuals transitioning out of TMM services are connected to sustained support that promotes long-term housing stability, recovery, and well-being.

ESSENTIAL DUTIES & RESPONSIBILITIES

Mental Health Programming & Service Delivery

  • Develop, implement, and facilitate trauma-informed mental health support groups, psychoeducation workshops, and wellness programming for residents and program participants.
  • Provide individual and group-based mental health consultation and support, utilizing evidence-based and culturally responsive approaches.
  • Assess program participants’ behavioral health needs and coordinate appropriate referrals and service plans.
  • Design curricula and programming tools that address co-occurring mental health and substance use challenges within the context of homelessness and housing instability.

Aftercare & Post-Exit Support

  • Build and manage TMM’s aftercare framework, including structured follow-up protocols, alumni engagement strategies, and warm handoff procedures to community-based providers.
  • Track post-exit outcomes and maintain ongoing contact with program graduates to identify emerging needs and reduce returns to homelessness.
  • Develop partnerships with housing providers, outpatient mental health clinics, peer support networks, and recovery organizations to create a robust continuum of care beyond TMM’s walls.

External Agency Liaison & Coordination

  • Serve as TMM’s primary point of contact with the Los Angeles County Department of Mental Health (LACDMH), contracted behavioral health providers, ICMS teams, and substance use treatment agencies.
  • Represent TMM in interagency meetings, coalitions, and community forums related to behavioral health, homelessness, and service coordination.
  • Identify and cultivate new partnerships that expand access to mental health resources for TMM participants and alumni.
  • Coordinate cross-agency case consultation on high-acuity individuals to ensure continuity and quality of care.

Staff Consultation & Training

  • Provide clinical consultation and behavioral health guidance to frontline staff, case managers, and program teams navigating complex participant situations.
  • Develop and deliver staff training on topics including trauma-informed care, de-escalation, mental health first aid, motivational interviewing, and recognizing signs of crisis.
  • Support a culture of compassionate, dignity-centered service delivery across all TMM programs.

Data, Outcomes & Compliance

  • Track and report on mental health service delivery metrics, aftercare engagement rates, and post-exit outcomes.
  • Contribute data and narrative content to grant applications, funder reports, and Board presentations in collaboration with the Director of Advancement and Director of Programs.
  • Ensure all mental health programming complies with applicable licensing standards, confidentiality requirements (including 42 CFR Part 2 and HIPAA), and organizational policies.
Qualifications:

QUALIFICATIONS

Required

  • Master’s degree in Social Work (MSW), Psychology, Counseling, or a closely related field.
  • Associate-level clinical licensure in the State of California (ASW, AMFT, APCC) or full licensure (LCSW, LMFT, LPCC).
  • Minimum of five (5) years of progressive experience in behavioral health, homeless services, or social services programming, with at least two (2) years in a program management or supervisory capacity.
  • Demonstrated expertise in trauma-informed care, harm reduction, and working with individuals experiencing homelessness, serious mental illness, and/or substance use disorders.
  • Experience coordinating with external agencies, government systems, and community-based organizations.
  • Strong facilitation, training, and group curriculum development skills.
  • Proficiency with HMIS, electronic health records, and outcomes tracking systems.

Preferred

  • Full clinical licensure (LCSW, LMFT, LPCC) or licensure expected within 12 months of hire.
  • Direct experience serving individuals on Skid Row or in the greater Downtown Los Angeles area.
  • Experience with Drug Medi-Cal or other Medi-Cal behavioral health billing structures.
  • Bilingual (English/Spanish) strongly preferred.

WORKING CONDITIONS

This position is based at The Midnight Mission’s main campus in the Skid Row community of Downtown Los Angeles. The role requires regular in-person presence and may involve direct engagement with individuals in crisis. Some evening or weekend availability may be required for programming, community events, or emergency response. Travel to partner agency sites within Los Angeles County is expected.

 

Salary : $95,000 - $130,000
