Alcott, a nonprofit organization dedicated to providing mental health and supportive housing services to low- and no-income adults across Los Angeles County, is seeking a motivated and compassionate Case Manager – ODR (ICMS) to join our Intensive Case Management Services team in partnership with the Los Angeles County Office of Diversion and Reentry (ODR) and Department of Health Services (DHS).

This role supports individuals exiting or at risk of involvement with the Los Angeles County jail system by providing intensive, field-based case management focused on housing placement, stabilization, and community reintegration. Work Setting: Field-Based, On-Site
Schedule: Full-Time, Monday through Friday, 8:30 a.m. – 5:00 p.m.
Pay Range: $24.00 – $26.00 per hour, DOE

What You’ll Do: The Impact You’ll Make
As a Case Manager – ODR, you will provide intensive outreach, engagement, housing navigation, and service coordination to adults experiencing chronic homelessness, mental illness, substance use disorders, and complex medical conditions. Working throughout Los Angeles County, you will help participants secure permanent supportive housing, connect to health and behavioral health services, and maintain long-term housing stability.

Outreach, Engagement, and Housing Navigation
• Conduct outreach in collaboration with DHS, the Los Angeles County Sheriff’s Department, Probation, and DMH to individuals referred from the jail system.
• Engage participants with chronic health conditions or high utilization of jail and emergency services to determine eligibility for permanent supportive housing.
• Complete pre-authorization packets and housing application materials in compliance with DHS policies.
• Assist participants with rental subsidies and housing searches, including coordinating with landlords and property managers.
• Travel throughout Los Angeles County to provide housing navigation and case management services.

Case Management and Service Coordination
• Coordinate mental health, medical, substance use, and recovery support services for an assigned caseload of approximately 15 participants.
• Ensure linkage to primary care and behavioral health services to reduce reliance on emergency and jail-based care.
• Develop individualized service plans with participants and update plans quarterly or upon goal completion.
• Provide crisis intervention, stabilization, and ongoing tenancy support as needed.

Documentation and Compliance
• Maintain complete, accurate, and current participant files in accordance with DHS and agency requirements.
• Document all services in the electronic health record in a timely and accurate manner.
• Support program compliance with contract, reporting, and auditing standards.

What You’ll Bring: Your Qualifications
• At least one (1) year of experience working with individuals experiencing homelessness and a bachelor’s degree in social work, psychology, or a related mental health field, or equivalent experience.
• Experience working with individuals with mental illness, chronic medical conditions, and substance use disorders.
• Knowledge of chronic homelessness, outreach and engagement strategies, intensive case management models, crisis intervention, suicide assessment and prevention, public benefits, affordable housing systems, landlord-tenant rights, and eviction prevention.
• Ability to perform required computer functions for electronic health record documentation.

Requirements
• Clearance through the Los Angeles County Sheriff’s Department for jail access.
• Valid California Driver’s License, clean driving record, and insurability under Alcott policy.
• Live Scan fingerprint clearance required.
• COVID-19 vaccination and one (1) booster required unless an approved medical or religious exemption is provided.
• If driving a company vehicle is an essential function, employee must maintain eligibility throughout employment.

Preferred Qualifications
• Experience collaborating with multidisciplinary teams and county partners.
• Bilingual in English and Spanish.
