Community Care Hospice is currently seeking a dedicated and skilled Licensed Practical Nurse (LPN) to provide high-quality hospice care in a home setting. This position plays a key role in supplementing the nursing care provided by the RN and ensures the delivery of patient-centered, end-of-life care with dignity, empathy, and professionalism.
Employment Type: Full Time | M-F 8am-5pm
Pay Rate: $26/hr (pay is based on qualifications and experience)
Location: Newton, KS
Key Responsibilities:

Deliver direct patient care under the guidance of a Registered Nurse and in alignment with the patient’s plan of care.

Monitor, report, and document changes in patient condition; collaborate with the interdisciplinary team.

Educate patients, families, and caregivers on care techniques and safety measures.

Assist with personal care activities including hygiene, mobility, and basic home maintenance.

Qualifications

Current LPN license in the state of Kansas.

Minimum of one year experience with chronically ill and/or terminally ill patients.

Valid Kansas Driver’s License and proof of auto liability insurance.

Reliable transportation and an acceptable Motor Vehicle Record per company standards.
