LPN (Licensed Practical Nurse) - Hospice

Job Summary:

Faith Home Health & Hospice is seeking compassionate, skilled, and dedicated Licensed Practical Nurses (LPNs) who are committed to providing exceptional end-of-life care with professionalism, dignity, and respect. As a Hospice LPN, you will deliver skilled nursing services, treatments, and preventive care while supporting patients and their families through education, guidance, and compassionate care in the comfort of their homes.

We are currently hiring Full-Time Hospice LPNs in the Wichita, Kansas area. Specific coverage territories will be discussed during the interview process.

Essential Responsibilities

As a Hospice Licensed Practical Nurse (LPN), you will:

• Provide hospice nursing services, treatments, and preventive care as assigned.
• Assess patient conditions and accurately communicate and document pertinent clinical information.
• Educate and counsel patients, family members, and caregivers regarding nursing care needs and disease management in the home setting.
• Participate in on-call rotations, including evenings, weekends, and holidays as assigned.
• Collaborate effectively with physicians, patients, families, and the interdisciplinary care team to ensure coordinated, high-quality patient care.
• Support patients and families with compassion, empathy, and professionalism throughout the hospice journey.

LPN QUALIFICATIONS:

• Current, valid, and unencumbered Kansas State Board of Nursing LPN license.
• Minimum of one (1) year of hospice nursing experience required.
• Valid driver's license, reliable transportation, and current automobile insurance.
• Current CPR certification or ability to obtain certification upon hire.
• Strong communication, assessment, documentation, and patient education skills.
