The Home Health Coordinator provides leadership and day-to-day operational oversight for Midland’s Home Health program. This role combines clinical excellence with people leadership—ensuring high-quality, compliant patient care and supporting our nurses in delivering exceptional outcomes.
You’ll serve as the Clinical Manager for the Home Health team, coordinating patient care, supervising staff, overseeing compliance, and ensuring every plan of care reflects the dignity and individuality of the people we serve.
What You’ll Do
- Lead and support the Home Health team in delivering compassionate, high-quality care.
- Oversee patient assignments, care coordination, referrals, and plan-of-care development.
- Ensure compliance with federal/state regulations, accreditation standards, and agency policies.
- Review OASIS documentation, billing accuracy, and Quality Reporting Program data.
- Facilitate interdisciplinary team (IDT) meetings and case conferences.
- Partner with Quality and Compliance to strengthen clinical and operational performance.
- Support staff development through education, coaching, and performance feedback.
- Monitor productivity, budgets, and utilization of clinical resources (pharmacy, DME, supplies).
- Collaborate with community partners and healthcare providers to expand program visibility.
- Step in for the Administrator when they are unavailable.
What We’re Looking For
Required
- Current, unrestricted Registered Nurse (RN) license in Kansas.
- Minimum 1 year of professional nursing experience.
- Experience with home health billing processes.
- Strong collaboration, critical thinking, and interpersonal skills.
- Valid Kansas or Missouri Driver’s License, reliable transportation, and insurable MVR.
- 2 years of home health or hospice experience.
- Leadership or management experience in a Medicare-certified home health program.
- Must be able to stand, walk, bend, lift (up to 50 lbs occasionally), and travel to patient homes.
- Primarily an in-person role in clinical and office settings.
- Dress Code: Scrubs or Business Casual.
