Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All-Inclusive Care for the Elderly) program. PACE Program

The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.

PACE Center Administrator

A PACE Center Administrator at Midland Care oversees the operations of a PACE (Program of All-Inclusive Care for the Elderly) center, serving as a bridge between clinical staff, administrative teams, and participants. This includes collaborating in all aspects which consist of a primary care medical clinic, 24-hour care delivery, preventive services, a provider network, therapeutic services, a day health center, home care, transportation, and performance improvement activities. The Center Administrator will create agendas, coordinate, and facilitate IDT meetings while providing a summary of Center operations and information on functional status as provided by the IDT, including monitoring center attendance and cancellations. Additionally, the Center Administrator collaborates with support departments when working with community providers/ contracts/ vendors. Duties include ensuring compliance with the Centers for Medicare and Medicaid Services’ (CMS) PACE guidelines. They ensure the center runs smoothly, manage resources and staffing, support quality and regulatory compliance, and create a welcoming, coordinated environment so seniors receive comprehensive, person-centered care.

People

  • Provides full range, direct supervision to direct reports including, Support/Operations Specialist, Life Enrichment Specialist, Center Certified Nurse's Aide/Med Aide and Home Health Aide, Companion Aide, Intake Coordinators and Registered Dietician.
  • Makes recommendations to higher level authority that carry weight about employment actions including hiring, discipline, pay increases/decreases and termination. Provides guidance, direction, feedback, counsel, and training to subordinate staff as needed. Completes annual reviews in a timely manner. Reviews and approves/denies requests for time off. Utilizes Midland Care’s progressive discipline process as needed.
  • Responsible for providing regular feedback through one-on-one meetings. Ensures those that this position supervises understands what is expected of them to be successful in their position. Documents relevant conversations and disciplinary actions in the Journal section of Paylocity.
  • Meets regularly with direct reports to review goals, trends, and performance measures to celebrate successes and to facilitate change as needed.
  • Uses effective interpersonal skills that promote interdisciplinary collaboration and communication throughout the organization.
  • Oversees the scheduling of adequate staff to meet participant/family needs including selection and training of new employees.
  • Supervises and maintains qualified staff, ensuring that appropriate personnel perform their functions within the organization, and staff and contract providers are trained on all policies and procedures.
  • Ensures contracting providers are aware of and adhere to the organization’s policies and procedures. Defines and informs staff of the relationship of PACE services to Midland Care Connection.
  • Models’ attendance at Midland events and in-services. Service
  • Ensures effective facilitation of interdisciplinary team (IDT) meetings for the coordination of activities, nutrition, personal care, clinic services, transportation, rehabilitation, and other services. Ensures that IDT members are documenting appropriately and timely to meet regulations and support Care Plans.
  • Assists in the coordination of 24-hour care delivery
  • Ensures the participant enrollment process is well-coordinated for a smooth transition into PACE.
  • Fulfills administrative requirements such as completion of forms as necessary, including daily participation reports, statistical records, and data forms, charts entries in a timely and accurate manner.
  • Performs all other reasonably related jobs as assigned.
  • Maintains a cooperative and supportive work relationship with participants, family, and program staff.
  • Serves as an active member of assigned committees, which may include Utilization Review, Safety Committee, QAPI Council, Procedure Review Team, and Senior Services Solutions.
  • Maintains the confidentiality of all company procedures, results and information about participants, or families.

Quality

  • Assures compliance with appropriate regulations and standards of care. Participates in survey processes.
  • Ensures compliance with policies and procedures.
  • Ensures effective communication with contracting providers to include education/orientation to PACE Programming.
  • Ensures that all regulatory reporting requirements are completed timely and accurately.
  • Ensures that participants are oriented to the program and have opportunities to express their opinions about the care and services they receive.
  • Ensures that all concerns/grievances are responded to in a timely manner.
  • Responsible for overseeing process improvement projects for assigned areas. Growth
  • Completes annual mandatory education requirements.
  • Represents Midland Care at state and regional industry association meetings/conferences.
  • Develops partnerships within the community and educates community members, healthcare providers and educational institutions about Midland Care philosophy and services.
  • Empowers staff to develop community relationships and provides education on Midland Care services.
  • Collaborates with peers to move Midland Care toward its operational and strategic goals.

Finance

  • Utilizes company resources in a prudent and appropriate manner. Is a good steward of company resources.
  • Utilizes problem solving skills required in conjunction with program staff with the goal of providing the highest level of quality care in the most cost-efficient model possible. Problem solving will be necessary to guide the care delivery team to the most appropriate decision that will meet both requirements of cost efficiency and quality.
