Position Summary
Provide direct services to patients as part of a multidisciplinary team. Provide ongoing, individualized care to patients in the inpatient unit including discharge planning.
Essential Duties & Responsibilities
Direct patient contact:
- Completes social work/psychosocial assessments on newly admitted patients. Reviews the assessments completed by the other inpatient social workers.
- Ensures goals and an individual plan of care are established for patients by the inpatient social workers.
- Collaborates with individuals, families, health system and community based interdisciplinary professionals on the plan of care.
- Ensures the timely development of the discharge plan for patients by the inpatient social workers. Assist with complex discharges.
- Implements individual or group therapies.
- Completes meetings with the patient’s support person(s).
- Ensures timely completion of documentation by the inpatient social workers in the medical record. Serves as the point of contact for State of Connecticut agencies such as the Department of Mental Health and Addiction Services and the Probate Court.
- Leads the daily treatment team meeting.
- Assist with monitoring quality indicators specific to inpatient social work.
- Provides clinical oversight of the social workers and social work students on the inpatient behavioral health unit. Reviews their work for completeness and accuracy, assists with the resolution of patient care conflicts.
- Completes insurance reviews when indicated.
- Completes timely charting in the electronic medical record.
Minimum Qualifications
- Master’s degree in social work from an accredited social work program
- Possession of valid clinical license (LCSW) in Connecticut
- The equivalent of three years of post master’s experience as a behavioral health/mental health clinician providing clinical and/or supervisory services.
- Employed as a social worker at Middlesex Health for 2 years
- Ability to diagnose using DSM 5
- Must have excellent writing and communication skills, be highly organized
- Must be able to work as a part of a team
Preferred Qualifications
- Previous experience providing clinical care and implementation of groups on an adult inpatient unit behavioral health unit
- SIFI training for MSW students
- Additional certifications in specialized clinical care such as DBT, CBT, trauma informed care, etc.
- Experience/knowledge of substance use disorders and treatment of patients with a dual diagnosis.
- Experience/knowledge working with LGBTQ population
