Overview
We are seeking a dedicated and experienced Transition Coordinator to join our team. This role is vital in supporting individuals with developmental disabilities as they transition to new phases of life, such as employment, education, or community integration. Duties

  • Develop and implement individualized transition plans tailored to each person's needs and goals.
  • Coordinate services across multiple agencies, including educational institutions, healthcare providers, and community organizations.
  • Manage program activities, ensuring compliance with relevant policies and regulations.
  • Supervise staff and volunteers involved in transition services, providing guidance and support.
  • Build and maintain strong relationships with clients, families, service providers, and community partners.
  • Conduct assessments to identify needs related to behavioral health, social skills, employment, or education.
  • Maintain detailed documentation of progress and outcomes for reporting purposes.
  • Provide training and support to team members on best practices in program management and behavioral health interventions.
  • Responsibilities
  • Answer calls and assist walk-ins with information and referrals.

Essential Functions

  • Communicate clearly (phone, email, in person) with consumers, staff, and partners.
  • Conduct group presentations.
  • Understand IL and PCM program requirements to guide or refer consumers.
  • Travel within and outside the service area as needed.
  • Accurately enter data into the database.
  • Operate office equipment (computers, calculators, etc.).
  • Sit for extended periods.
  • Maintain strict confidentiality.

Preferred Qualifications

Education

  • High School diploma or GED required.
  • Associate degree in human services or related field preferred.
  • Relevant experience may substitute for education.

Experience

  • 2–4 years in a related role.
  • Proficient in Microsoft Office and database management.
  • Bilingual (Spanish) preferred.

Skills

Knowledge

  • Independent Living philosophy.
  • Community resources and service systems for people with disabilities or older adults.
  • MassHealth and state agency programs.

  • Gather details to check eligibility for programs and services.
  • Provide backup support at the front desk when needed.
  • Attend trainings to improve knowledge and service quality.
  • Represent CLW at meetings and outreach events.
  • Enter data into the I&R database.
  • Follow up within 48 hours on calls or messages.
  • Deliver Independent Living (IL) Core Services as requested.
  • Help with marketing and outreach activities.
  • Provide interpretation when needed.
  • Assist with special projects like mailings.
  • Keep accurate records and files per policy.
  • Perform other duties as assigned.

Qualifications

  • Proven experience in management, program development, or program management within social work or related fields.
  • Supervising experience working with individuals with developmental disabilities or behavioral health needs is highly desirable.
  • Strong relationship management skills with the ability to collaborate effectively across agencies and with diverse populations.
  • Administrative experience including budgeting, documentation, and compliance oversight.
  • Knowledge of social work principles, behavioral health strategies, and community resources.
  • Excellent organizational skills with the ability to manage multiple priorities efficiently.
  • Compassionate approach with a commitment to empowering individuals through supportive transition services.
  • Bachelor's degree in social work, psychology, public administration, or a related field; advanced degrees are preferred. This position is integral in helping individuals achieve independence and success through well-coordinated transition planning. We value candidates who are proactive, empathetic, and dedicated to improving lives through effective program management and relationship building.

Pay: $14.50 - $15.50 per hour

Expected hours: 30.0 per week
