Job Overview
We are seeking a dedicated and compassionate Clinical Liaison to join our Healthcare team Southeast Texas area. The Clinical Liaison will serve as a vital link between patients, families, and healthcare providers, ensuring that patients receive the highest quality of care. This role requires strong communication skills, clinical knowledge, and the ability to build relationships with various stakeholders in the healthcare environment.
Duties
- Act as the primary point of contact for patients and families, providing information about services and care options.
- Collaborate with healthcare providers to develop and implement individualized care plans for patients.
- Conduct assessments to determine patient needs and facilitate appropriate referrals to services.
- Educate patients and families about treatment options, resources, and support services available.
- Maintain accurate documentation of patient interactions and care plans in compliance with regulatory standards.
- Participate in interdisciplinary team meetings to discuss patient progress and care strategies.
- Build and maintain relationships with community organizations, hospitals, and other healthcare providers to enhance patient access to services.
Qualifications
- Bachelor’s degree in Marketing or related field preferred. Nursing, Social Work, or a related field: relevant clinical experience may be considered.
- Previous experience in a clinical liaison or similar role is highly desirable.
- Healthcare Marketing experience.
- Strong understanding of healthcare systems, patient care processes, and community resources.
- Excellent interpersonal skills with the ability to communicate effectively with diverse populations.
- Ability to work independently as well as part of a collaborative team.
- Proficient in using electronic health records (EHR) systems and other relevant software applications.
- A valid driver’s license may be required for travel between facilities or community locations.
