Position : Lamphere Center Transition Coordinator
Date Available: 2026/2027 SY
Salary: At the appropriate step on the salary schedule
Plus $7,500 annual stipend
Transition Coordinator Approval
Minimum Qualifications:
- Certified teacher in an area of Special Education (Cognitive Impairment Endorsement or Autism Spectrum Endorsement Preferred)
- Master’s Degree in an area of Special Education or Educational Leadership preferred
- At least 5 years of satisfactory teaching experience in an area of Special Education
- At least 3 years of satisfactory teaching experience of special education students in middle school, high school, or post high
- Experience in development of the transition section of IEPs and demonstrated familiarity with community agencies.
- Strong consultative and communication skills
- Demonstrated leadership skills
Job Requirements:
- Serve as the transition coordinator for the district
- Attend Oakland County Transition meetings
- Assist Parents with transition training/post graduate/ center opportunities
- Hold department meetings at the secondary level regarding transition work
- Attend Adult Transition Program Planning meetings at Oakland Schools
- Assist with helping parents/students with alternatives after graduation.
- Assist parents in connecting with outside agencies and supports.
- Assist with professional learning for transition assessments and components of the IEP
- Oversee PAES lab usage and materials.
- Serve as the Work Site-Based Learning Coordinator for the District Adult Transition Program
- Assist with creating schedules, completing the contracts, and completing any safety-related trainings at work site-based learning job sites
- Monitor the work site-based learning sites and assist job coaches with expectations
- Assist in finding new worksites
- Help facilitate problem-solving meetings for students
- Monitor Student Learning Objective Data
- Assist the staff and students with curriculum initiatives and increase opportunities for engagement
- Assist staff with creating academic data monitoring tools and strategies
- Assist with professional learning for staff and for paraprofessionals
- Take the lead on onboarding new paraprofessionals and other staff as necessary
- Attend debriefing meetings and classroom team meetings as necessary.
- Assist with student behavior as needed
- Attend IEP meetings as necessary
- Serve as a member of the Lamphere Center building leadership team
- Assist the director with overseeing committees at Lamphere Center.
- Any other responsibilities as designated by Director
This position is classified as Exempt
Employment is contingent upon disclosure in accordance with PA-189.
Ability to withstand the strains and successfully manage the responsibilities of the position.
Must demonstrate reliability and punctuality, with a strong record of consistent attendance. Regular and dependable presence during school hours is essential.
