Job Summary

The Social Services Coordinator is responsible for promoting the emotional, social, and psychosocial well-being of residents in a long-term care facility. This position serves as an advocate for residents and their families by coordinating services, assisting with care planning, arranging medical appointments, and ensuring residents have access to needed community resources. The coordinator works closely with the interdisciplinary team to enhance residents' quality of life while maintaining compliance with state and federal regulations.

Essential Duties and Responsibilities

  • Complete psychosocial assessments for residents upon admission and as needed.
  • Develop, implement, and update individualized social service care plans.
  • Provide emotional support, counseling, and crisis intervention to residents and families.
  • Assist residents and families with discharge planning and transitions of care.
  • Coordinate with physicians, hospitals, specialists, and community agencies to ensure continuity of care.
  • Schedule, confirm, and manage residents' medical, dental, vision, and specialty appointments.
  • Arrange and provide transportation for residents to and from medical appointments when required.
  • Ensure all appointment details, transportation arrangements, and follow-up recommendations are documented in the resident's medical record.
  • Communicate appointment outcomes and physician recommendations to the nursing team and attending providers.
  • Assist residents with obtaining community resources, financial assistance programs, insurance information, and advance directives.
  • Participate in interdisciplinary care plan meetings and advocate for residents' psychosocial needs.
  • Maintain accurate, confidential documentation in accordance with HIPAA and facility policies.
  • Address resident concerns, grievances, and family issues in a timely and professional manner.
  • Organize and coordinate resident and family meetings as needed.
  • Collaborate with nursing, therapy, dietary, activities, and administration to promote resident well-being.
  • Ensure compliance with all federal, state, and facility regulations related to social services.

Qualifications

  • CNA certification.
  • Experience in long-term care, skilled nursing, healthcare, or social services preferred.
  • Valid driver's license with a safe driving record (required if transporting residents).
  • Excellent communication, organization, and interpersonal skills.
  • Ability to maintain confidentiality and exercise sound judgment.
  • Knowledge of Medicare, Medicaid, long-term care regulations, and community resources is preferred.
  • CPR certification may be required based on facility policy.

Physical Requirements

  • Ability to assist residents during transportation and appointments.
  • Ability to sit, stand, walk, bend, and lift up to 25 pounds as needed.
  • Ability to safely operate a facility vehicle when transporting residents.

Work Environment

This position is performed in a long-term care facility with frequent interaction with residents, families, healthcare providers, and community agencies. Travel to medical appointments, hospitals, and specialist offices is required.

Pay: From $15.00 per hour
