What are the responsibilities and job description for the Case Manager/Care Coordinator position at Metropolitan Community Services?
Job Overview:
A local non-profit in-home care services agency is looking for an experienced Designated Coordinator to administer admission and supervision of our 245D Intensive Support Services.
Duties:
- Completing screenings/intakes for new clients
- Developing and/or writing and implementation of each client’s person-centered support plans including (SPA) Support Plan Addendum, (SMA) Self-Management Assessment, and (IAPP) Individual Abuse Prevention Plan.
- Staying in compliance with 245-D licensing rules and regulations.
- Performing quarterly, semi-annual and annual reviews as needed or directed by the person served and/or team in order to person-centered services.
- Recruitment and training of Direct Support Professionals (DSPs) for your assigned caseload.
Qualifications:
- Bachelor's degree in a field related to human services AND 1-year full-time experience providing direct care to individuals with disabilities or to those who are age 65 or older; AND
- At least two years of experience as a Designated Coordinator for intensive support services; OR
- An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
Additional Requirements:
- Successful completion of a background study conducted by the Minnesota Department of Human Services.
- Possession of a valid driver’s license, with a clean driving record, and reliable transportation for work-related travel.
- Ability to independently travel throughout the 7-county metro area.
