Overview
Position Summary
The Designated Coordinator is responsible for coordinating, supervising, and evaluating the delivery of person-centered services for individuals receiving 245D Home and Community-Based Services. This role ensures compliance with Minnesota Statute 245D, supports quality service outcomes, and promotes the health, safety, rights, and independence of persons served.
Key Responsibilities:
* Coordinate and oversee service delivery for individuals receiving 245D HCBS services in accordance with support plans and person-centered goals.
* Monitor implementation of individual support plans, assessments, and outcomes to ensure services meet identified needs.
* Supervise and support direct support staff to ensure quality care, competency, and compliance with agency and DHS standards.
* Conduct regular service reviews, documentation audits, and progress evaluations for each person served.
* Ensure all required documentation is accurate, current, and compliant with Minnesota DHS 245D regulations.
* Collaborate with case managers, legal representatives, families, guardians, and interdisciplinary teams regarding service planning and updates.
* Assist in development, revision, and implementation of individual service plans and addendums.
* Ensure staff training, orientation, and ongoing competency requirements are completed and documented.
* Maintain confidentiality and uphold the rights of persons served under Minnesota HCBS standards.
* Participate in licensing reviews, audits, and quality assurance activities.
Qualifications:
Candidate must meet Minnesota 245D Designated Coordinator qualifications under Minn. Stat. §245D.081, including one of the following:
1. Bachelor’s degree in human services, education, health, or related field 1 year relevant direct care experience; OR
2. Associate degree in related field 2 years relevant direct care experience; OR
3. Diploma in related field 3 years relevant direct care experience; OR
4. Minimum 50 hours of education/training in human services and disabilities 4 years relevant direct care experience under qualified supervision. ([MN Revisor's Office][1])
Preferred Skills:
* Knowledge of Minnesota 245D licensing regulations and DHS requirements

Reports To: Designated Manager / Program Director
Work Environment:
This position may involve office work, community visits, staff supervision, and travel to client homes or service locations as needed.
Pay: $23.00 - $25.00 per hour
