Life Fountain is currently seeking a motivated and compassionate entry-level professional to join our home healthcare team as a Supervisor. Our agency is currently licensed to provide Personal Care Assistance (PCA), Community First Services and Supports (CFSS), and 245D services.

A professional in this role would be responsible for supervising direct care staff providing PCA/CFSS and 245D services. As a Supervisor, you will play a crucial role in ensuring that service recipients receive the care they need by coordinating staffing and service activities, hiring and training staff, and addressing any concerns from recipients or staff. The Supervisor would be serving as a QP/SP and a DC to ensure coverage for PCA, CFSS, and 245D services.

Qualified Professional (QP)/Supervising Professional (SP)

As a QP/SP, the Supervisor will be responsible for coordinating and ensuring the delivery of PCA and CFSS services based on a qualified home care assessment. This includes supporting recipients as they transition from PCA to CFSS for ongoing home care. QP/SP will ensure compliance with applicable service and staffing requirements for either service, depending on the recipient’s current program needs. QP/SPs must also ensure staff are trained and competent to provide the services. QP/SPs must remain knowledgeable and up to date on PCA/CFSS services to maintain supervisory responsibility for services and staff/workers.

Designated Coordinator (DC)

As a DC, the Supervisor will be responsible for coordinating 245D services based on referrals and supporting documentation received. DCs are responsible for overseeing the administration of a recipient’s 245D home care services, including completing required documentation, setting up and attending service and support meetings, and conducting staff training as necessary to ensure services meet the recipient's needs. DCs must stay up to date on program and service-specific functions to ensure staff competency through training.

Qualifying Education and Experience
Candidates must meet one of the following:

  • Bachelor’s degree in a field related to human services, education, or health, plus 1 year of full-time (2,000 hrs/year) direct care experience with people with disabilities or adults 65 .
  • Associate degree in a related field, plus 2 years of full-time direct care experience with people with disabilities or adults 65 .
  • Diploma in a related field, plus 3 years of full-time direct care experience with people with disabilities or adults 65 .
  • 50 hours of education/training in human services and disabilities, plus 4 years of supervised full-time direct care experience with people with disabilities or adults 65 .

Required Skills and Experience

  • Bilingual: English/Spanish
  • Supervisory/Leadership experience in a social service program
  • Practical oral, written, and communication skills
  • Experience with the administration of the 245D licensing requirement
  • Reliable transportation for work driving purposes
  • Leadership and management skills, including time management, organization, and prioritization skills
  • Familiarity with HIPAA and Data Privacy laws
  • Receptive to learning and applying new skills, techniques, and approaches
  • Demonstrated flexibility, anticipates, and responds effectively to change
  • Ability to consistently exercise discretion and independent judgment
  • Practical computer technology skills and ability to learn how to use agency software, including but not limited to: Microsoft Office Applications (Word/Excel/Outlook/SharePoint, etc.), various web applications, GPS navigation, text messaging, Printer/Fax/Scanner, etc.

Job Type: Full-time

Pay: $20.00 - $24.00 per hour
