Responsibilities:


• Assist clients in obtaining vital records, including birth certificates, Social Security cards, driver's licenses, and state identification cards.
• Assist clients with applying for and maintaining benefits such as Medicaid, ARHOME/insurance coverage, SNAP, WIC, TEA/TANF, SSI/SSDI, and other community assistance programs.
• Coordinate referrals to community agencies and assist clients in accessing employment, education, childcare, and other supportive services.
• Coordination with the office manager regarding client appointments, medical providers, specialists, DHS, probation/parole, and other community resources.
• Transport clients safely to appointments, benefit offices, and other approved activities using agency vehicles.
• Maintain accurate documentation of all case management activities in accordance with agency policies and state licensing requirements.
• Communicate regularly with the treatment team regarding client progress and identified needs.
• Assist clients in developing problem-solving skills and increasing independence in accessing community resources.
• Advocate on behalf of clients when appropriate to ensure continuity of care and access to needed services.
• Maintain confidentiality in accordance with HIPAA, 42 CFR Part 2, and agency policies.
• Participate in staff meetings, training, and supervision as required.
• Perform other duties as assigned by the Program Director, Administrator, Medical Director or CEO.

Minimum Qualifications:
• Must be at least twenty-one (21) years of age
• High school diploma or GED required; Associate's or Bachelor’s degree in Social Work, Human Services, Psychology, or a related field preferred.

• Experience working with individuals experiencing substance use disorders, mental health disorders, or co-occurring disorders preferred.
• Knowledge of community resources and public assistance programs.
• Strong organizational and time management skills.
• Excellent communication and interpersonal skills.
• Ability to establish professional, supportive relationships with clients from diverse backgrounds.
• Ability to maintain confidentiality and professional boundaries.
• Basic computer proficiency with the ability to complete professional documentation.
• Valid driver's license with an acceptable driving record.
• Ability to pass criminal background and drug screening requirements.

Preferred skills:
• Experience working in residential treatment or behavioral healthcare.
• Knowledge of Medicaid eligibility and public benefit application processes.
• Familiarity with electronic health records (EHR).
• Ability to de-escalate stressful situations and work effectively with clients experiencing crisis.
• Strong advocacy and resource coordination skills.

Physical Requirements:
• Ability to sit, stand, and walk, for extended periods of time
• Must be able to maintain effective audio, visual discrimination and perception needed for making observations, communicating with others, reading and writing, and operating office equipment and agency vehicles
• Ability to safely transport clients using agency vehicles.
• Ability to work in a residential treatment environment with occasional exposure to emotionally challenging situations.

Work Environment:

This position works in a residential treatment facility serving women with co-occurring substance use and mental health disorders who reside with their children. The work requires flexibility, professionalism, compassion, and the ability to manage multiple priorities while supporting clients through the recovery process.

Case Manager Job Description-Arkansas CARES
When an employee performs two or more different jobs, for which different straight time hourly rates are established, the employee will be paid during overtime hours at a rate not less than one and one-half time the hourly rate established for the type of work he or she is performing during the overtime hours.

Pursuant to the Arkansas Medical Marijuana Act 593, this position is a designated safety-sensitive position according to MFH/MCH/MBH standards and processes.

Level One - Full Access: Ongoing regular access to PHI of all forms while the employee is on duty and performing within the scope of his or her job as defined by the employee's job description, and Policy and Procedure. "Such access must be for cause, consistent with job responsibilities and related to patients, claims, audits, reviews and other legitimate business purposes." (e.g. Physicians, nurses and other clinicians)
