Metairie Health Care Center, a long-term care skilled nursing facility is seeking a Social Services Director.
This position will include the following:
- Evaluate patients on their needs, limitations and desires
- Address patient concerns and goals while maintaining constant communication with the patients and their families
- Build rapport with patients and their families
- Work with an interdisciplinary team to create care plans, address psycho-social issues and promote the quality of life
- Have excellent listening and communication skills
A Bachelor's or Masters Degree in social work or other human services field is required.
Two or more years of clinical social work experience is preferred.
