Outpatient Dietitian – Diabetes Education

This is an opportunity for a credentialed nutrition or nursing professional to provide specialized diabetes education and medical nutrition therapy in an outpatient setting. The role will help patients and families develop practical strategies for managing diabetes and related health conditions while supporting broader nutrition, wellness, and disease-management goals. The ideal candidate will bring strong diabetes education expertise, excellent communication skills, and a patient-centered approach to counseling and care.


Key Responsibilities

  • Provide individualized medical nutrition therapy and nutrition counseling to patients and families.
  • Deliver diabetes self-management education and support in accordance with established clinical standards and regulatory requirements.
  • Assess patients' nutritional needs through diet histories, exercise habits, health information, and relevant social or emotional factors.
  • Obtain and document patient measurements, including weight, height, blood pressure, and heart rate as appropriate.
  • Counsel patients and families regarding nutrition, dietary restrictions, diabetes management, weight management, cardiovascular health, renal disease, gastrointestinal conditions, and other nutrition-related needs.
  • Register patients for diabetes self-management education classes and coordinate referrals and appointments.
  • Obtain relevant medical and insurance information from referring providers and follow up on required referrals.
  • Maintain accurate and timely documentation in the electronic health record.
  • Develop and utilize evidence-based nutrition education materials and resources.
  • Update outpatient dietitian documentation templates and educational resources as needed.
  • Collaborate with inpatient clinical staff on nutrition and diabetes-related issues.
  • Support initiatives designed to increase patient referrals and community awareness of diabetes education services.
  • Participate in departmental, hospital-wide, and community education and professional development activities.


Qualifications & Experience

  • Registered Dietitian (RD) or Registered Nurse (BSN) required.
  • Master's degree preferred.
  • Certified Diabetes Care and Education Specialist (CDCES), formerly Certified Diabetes Educator (CDE), required.
  • Minimum of 1 year of diabetes education experience required.
  • At least 1 additional year of experience in a related healthcare field preferred.
  • Demonstrated knowledge of diabetes education, medical nutrition therapy, and patient self-management principles.
  • Ability to work effectively with patients, families, providers, and multidisciplinary teams.
  • Strong organizational, analytical, planning, and decision-making skills.
  • Strong written and verbal communication skills.
  • Experience developing or delivering diabetes self-management education programs preferred.

Work Environment & Benefits

The position is based in a 160-bed, not-for-profit medical center and reports to the Lead Dietitian. The role offers an opportunity to combine clinical nutrition expertise with diabetes education while working collaboratively with outpatient, inpatient, and community healthcare teams.


Compensation: Approximately $50 per hour midpoint, based on qualifications and experience.
