Clerical Assistant – Inpatient Care Management
Part-time, 3 days per week 8-4:30, weekend and holiday rotation, 48 hours bi-weekly

As a Clerical Assistant in Inpatient Care Management, you’ll support the team in facilitating safe, timely transitions for hospitalized patients. From scheduling follow-up appointments to arranging transportation and delivering regulatory notices, you’ll play a key role in ensuring continuity of care and compliance with quality standards.

Key Responsibilities

Perform general office and secretarial duties to support care coordination and discharge planning

Deliver the Important Message to Medicare beneficiaries and assist with the appeal process when needed

Schedule post-discharge follow-up appointments to meet quality performance metrics

Meet with patients to educate them about appointments and identify barriers to attendance

Arrange patient transportation as indicated

Assist in the development and implementation of departmental projects and performance improvement initiatives (Katas)

Maintain accurate documentation and support efficient department operations

Minimum Qualifications
Education

High school diploma or equivalent required

Associate degree preferred
Experience
-
Previous clerical experience in a healthcare environment preferred

Experience with Microsoft Office, fax machines, and electronic medical records preferred
