Meridian is seeking a Case Manager for our Residential program
Starting rate is $15-$18/hour and is based on education and experience.
In addition to the hourly rate, our essential staff is also receiving an additional $3.40/hour incentive pay when working on site.
Schedule is Monday-Friday 8:00am- 5:00pm
Position Overview:
The Case Manager is responsible for managing a caseload of clients with substance use history and for assessing client needs, developing, implementing and reviewing treatment plans, and working with other community resources to help meet/achieve client service needs.
Principal Duties and Responsibilities (Essential Duties):
1. Complete discharge plans and summaries;
2. Participate in treatment team’s case review;
3. Consult with other professionals in support of client’s treatment;
4. Assist staff and clients with community resources, including housing and aftercare and services/documentation;
5. Assist with Medicaid applications;
6. Other duties as assigned.
Qualifications:
- Education: Bachelor’s degree in appropriate academic discipline (e.g. Social Work, Psychology, Criminal Justice) preferred.
- Experience: A minimum of one to three (1-3) years of case management experience or other relevant experience in behavioral health and/or treatment of substance use disorders preferred.

- Ability to read and write English legibly.

Physical Requirements:
- Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
- Must be able to lift and carry up to 50 lbs
- Must be able to talk, listen and speak clearly
