About the Role:
As a Care Coordinator, you'll support individuals with complex needs by assessing, planning, and coordinating services-primarily for those with opioid use disorder. You'll connect clients to resources, monitor progress, and advocate for their continued care and recovery.
What You'll Do:
- Conduct comprehensive client assessments
- Develop and manage individualized service plans
- Coordinate services and community resources
- Monitor client progress through regular contact
- Maintain accurate documentation and records
Who We Are Looking For:
- Bachelor's degree in healthcare, psychology, social work, or related field
- 1 year of care management experience (SMI, SUD, or similar populations)
- Valid Florida driver's license with clean record
