Position Summary
Meridian is seeking a full-time Community Support Worker (CSW) located in Cibola County to support our expansion of community-based behavioral health services. The ideal candidate is deeply connected to their community, understands local resources, and is passionate about empowering justice-impacted, involved, and at-risk youth, adults, and families.
Responsibilities
- Provide direct support and skill-building services to clients in their homes and community settings.
- Coordinate with community partners, schools, and agencies to connect clients to needed resources.
- Support clients in developing life skills, self-advocacy, and independent living abilities.
- Document services in compliance with state and agency standards.
- Participate in weekly supervision via telehealth/video with a supervisor in Bernalillo County.
- Attend in-person meetings with the Meridian Lead Team once per month in Cibola County.
- Uphold Meridian’s mission, values, and culturally attuned approach to care.
Qualifications
- Minimum of a High School Diploma or GED; Bachelor’s degree preferred.
- Prior experience in community support, social services, or behavioral health strongly preferred.
- Familiarity with Cibola County resources and community dynamics.
- Strong communication, organization, and documentation skills.
Valid NM driver’s license, reliable transportation, and willingness to travel for monthly meetings.
