Overview:
The Nurse Navigator is responsible for coordinating the care of patients. This role utilizes the nursing process to provide patient care and maintains professional accountability for provision of patient care for the assigned patients. The goals of this position include evaluating the overall effectiveness of care provided by other direct caregivers, coordinating patient care in conjunction with other departments, maintaining patient rights and confidentiality of patient information.
Qualifications:
- Required Education: Graduate of an accredited registered professional nursing program.
- Licensure: Current RN license in Oklahoma.
- Certifications: Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire.
- Preferred Experience: RN case management experience.
- Physical requirements:
- Push, pull, and/or lift 50 pounds on a regular basis
- Prolonged standing and walking each shift
- Ability to grip, reach, bend, kneel, twist, and squat to perform duties
