Job Summary and Responsibilities
Position Summary:
The incumbent provides care, treatment and service to Home Health and Hospice patients and their families in their place of residence within their scope of practice. Functions as a member of the interdisciplinary team.
Job Requirements
Experience:
- Two (2) years LVN Experience within the last 5 years
- Recent experience as an LVN in a Home Health or Hospice Agency - preferred.
Licensure:
- Currently licensed as a LVN in California
- Current California driver's license, possession of automobile insurance, & access to an operating motor vehicle.
- Current CPR/BLS card approved through the American Heart Association.
