Mercy Home Health is a new and rapidly growing home health company, and we’re looking for RNs willing to provide our patients with top-quality care.

This position is for Home Health Field Nurse

Monday - Friday

The right candidate will be independent, ambitious, focused and detail-oriented. Prior home health experience and knowledge of Kinnser/Wellsky is a plus.

Requirements:

· Unencumbered Louisiana LPN license

· Valid driver’s license

· Active state-minimum car insurance

· Current CPR card

· TB test

· Mobile/Cell Phone

Job Description:

. Provides services to clients in accordance with the State Nurse Practice Act, Agency Policy, and accepted professional standards of practice.

. Performs comprehensive assessments of clients status, including physical, psychosocial, and environmental parameters.

. Develops and/or follows an individualized Plan of Care that incorporates analysis of data and current scientific findings.

. Administers medications

. Teaching skills according to the clients needs and within the context of the clients environment, value system, and physical/emotional status.

. Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).
