What You'll Do:

The PACE Home Care Coordinator (LPN) is an integral member of the PACE Interdisciplinary Team (IDT) and is responsible for conducting comprehensive home care assessments for new participants and ongoing reassessments for existing participants within the scope of practice of the Louisiana Licensed Practical Nurse and under the direction of the Registered Nurse, Primary Care Provider, Medical Director, or other licensed practitioner as permitted by Louisiana law.

The Home Care Coordinator collects, documents, and communicates assessment findings regarding skilled and unskilled home care needs, durable medical equipment (DME), adaptive equipment, and other assistive devices for review by the Interdisciplinary Team (IDT). The Home Care Coordinator coordinates the implementation of home care services and DME based on the participant's established plan of care and provider orders.

In collaboration with the IDT, the Home Care Coordinator assists in determining each participant's ability to be safely supported in the community through PACE services. The position provides participant, caregiver, and staff education related to home safety and disease management within the LPN scope of practice.

The Home Care Coordinator practices in accordance with the Louisiana Licensed Practical Nurse Practice Act, CMS PACE regulations, organizational policies, and the participant's individualized plan of care.

Essential Duties and Responsibilities

  • Serves as an active member of the PACE Interdisciplinary Team (IDT) and completes initial and ongoing home care assessments within the Louisiana LPN scope of practice.
  • Collects, documents, and communicates participant assessment findings to the IDT to assist with care planning and coordination.
  • Participates with the IDT in the development, implementation, and revision of participant home care plans consistent with the established plan of care and within the LPN scope of practice.
  • Coordinates home care services utilizing PACE staff and contracted providers in accordance with participant care plans developed by the IDT.
  • Observes, documents, and promptly reports changes in participant condition to the Registered Nurse, Primary Care Provider, Medical Director, or other appropriate IDT member for evaluation and intervention.
  • Coordinates the procurement, delivery, maintenance, repair, and follow-up of durable medical equipment (DME) and assistive devices as ordered by the licensed practitioner and approved through the IDT.
  • Assists with case management activities and care coordination during hospitalizations, skilled nursing facility stays, and other care transitions.
  • Communicates with hospitals, rehabilitation facilities, home health agencies, contracted providers, participants, and caregivers to facilitate safe transitions of care.
  • Assists the Home Care Manager, Registered Nurse, or Director of Clinical Services with orientation, competency validation, and continuing education of home care staff and contracted providers.
  • Provides participant, caregiver, and staff education regarding home safety, infection prevention, fall prevention, equipment use, and home care services within the LPN scope of practice.
  • Completes required documentation in the PACE Electronic Health Record (EHR) in accordance with CMS PACE regulations and organizational policy.
  • Participates in daily morning meetings, IDT meetings, quality improvement activities, and other organizational meetings as assigned.
  • Performs all duties in accordance with the Louisiana Licensed Practical Nurse Practice Act, CMS PACE regulations, organizational policies, and under the direction of the Registered Nurse, Primary Care Provider, Medical Director, or other licensed practitioner.
  • Performs other duties as assigned.

Competencies, Preferred Knowledge, Skills, and Abilities

  • Current, active, unencumbered Louisiana Licensed Practical Nurse license.
  • Current Basic Life Support (BLS) certification.
  • Experience in home health, hospice, community health, long-term care, or PACE preferred.
  • Experience coordinating home care services and durable medical equipment preferred.
  • Knowledge of CMS PACE regulations and interdisciplinary care planning preferred.
  • Valid Louisiana driver's license, reliable transportation, and proof of automobile insurance.
  • Proficiency with Electronic Health Records (EHR).
  • Demonstrates sound clinical judgment appropriate to the Louisiana LPN scope of practice.
  • Recognizes changes in participant condition and promptly communicates findings to the supervising Registered Nurse, Primary Care Provider, Medical Director, or other appropriate IDT member.
  • Excellent organizational, communication, and time-management skills.
  • Ability to work collaboratively as a member of an interdisciplinary team.
  • Ability to educate participants, caregivers, staff, and contracted providers within the LPN scope of practice.
  • Ability to maintain confidentiality and professionalism while working with frail older adults and their caregivers.

Reporting Relationship

The Home Care Coordinator (LPN) reports to the Director of Clinical Services or designee and practices under the direction of the Registered Nurse, Primary Care Provider, Medical Director, or other licensed practitioner as permitted by the Louisiana Licensed Practical Nurse Practice Act and CMS PACE regulations.

What You'll Bring:

Minimum Qualifications

  • Graduate of an accredited Practical Nursing program.
  • Current, active, unencumbered Licensed Practical Nurse (LPN) license in the State of Louisiana (Required).
  • One (1) year of experience working with a frail or elderly population, home health, community health, long-term care, hospice, or PACE preferred.
  • One year of experience working with a frail or elderly population, or completion of required PACE training prior to providing participant care (CMS required).
  • Demonstrated proficiency on a standardized set of core competencies for the PACE Home Care Coordinator position (CMS required).
  • Medically cleared for communicable diseases and current on required immunizations prior to participant care (CMS required).
  • Not excluded from participation in Medicare or Medicaid programs.
  • Successfully completes required background screening consistent with CMS and organizational requirements.
