Job Description Details:
Clinical Pharmacist, Ambulatory Care to support Mercy Health Physician Partners Primary and Specialty Care Network. Focus for this position is on supporting ambulatory program growth, supporting and innovating for population management strategies for pharmacotherapy programs, and limited (approximately 2 days/week) clinical practice in ambulatory/PCMH setting. PharmD and PGY-1 required (or equivalent experience). PGY-2 in Ambulatory Care and/or 3-5 year's experience in ambulatory care strongly preferred.
