Description:

▪ Reviews, prepares and dispenses medication orders per physician order or prescription according to established policies,

procedures, and protocols. Can work independently but stays within departmental policies and procedures.

▪ Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures and

protocols.

▪ Contributes to the quality and effective operation of the department.

▪ Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to

physicians, as needed.

▪ Provides drug information and counseling to patients, families, and other healthcare professionals.

▪ Participate in the training, education, precepting, and evaluation of pharmacy students and/or residents as assigned.

▪ Provides work direction to pharmacy support personnel, verifies the daily activities of pharmacy technicians.

▪ Performs other related duties as assigned.

Clinical Pharmacist, Population Health

  • Develops, coordinates, and manages programs designed to improve total cost of care outcomes for targeted value-based care

populations with a focus on medication and disease management, quality incentive program outcomes, and/or cost containment.

  • Utilizes data to proactively identify patients who may benefit from population health program intervention; coordinates with

Providers, care management, or other care team members to develop, document and execute drug therapy plans using safe,

effective, and economical medication therapy options.

  • Demonstrates knowledge and skill related to care coordination, population health, value-based contracting, as well as the design

and implementation of clinical programs.

Education

Minimum Five year bachelor of science degree in pharmacy from an accredited college of pharmacy.

Preferred Advanced training in at least one specialty clinical area.

Credentials/Licensure

Minimum Current registered pharmacist license and controlled substance license issued by the Michigan State Board of Pharmacy. Basic Cardiac Life Support (BCLS) certification may be required (at discretion of manager).

Related Experience

Minimum N/A

Preferred One year of experience in a hospital or retail pharmacy.

Other Knowledge, Skills And Abilities

Minimum Knowledge of drug regimen review, pharmacy software for order entry, allergy screening, drug interactions, and other related topics. Ability to communicate effectively.

Preferred Excellent written and verbal communication skills. Basic knowledge of Word, Excel and other computer applications.

Computer Competency

Familiarity with standard desktop and windows based computer system, including email, e-learning, intranet and computer navigation. Ability to use other software required to perform essential functions.

Physical/Mental

Minimum Repetitive use of hands and fingers (e.g., preparing IV admixtures, use of a computer keyboard). May require lifting and carrying light loads (up to 40 lbs.), including boxes, equipment, unit dose-cassettes, and IV solutions and stooping or kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves, and to file documents in lower file drawers). Sitting, walking, or standing for long periods of time (4-8 hours) is often necessary. Must be able to physically operate the equipment for the job.
