What are the responsibilities and job description for the Caseworker- Bi-lingual Spanish position at Mercy Drive, Inc.?
Mercy Drive, Inc serves people with developmental disabilities. We provide person-centered focused services that respond to the needs of our Individuals and their families/ Based on the assessment of each case, the caseworker will commence with casement management in our Day Habilitation Services or Residental Service department.
Caseworkers coordinate with the families in providing satisfactory services within the program and to ensure the individuals’ needs are met. Maintaining case records, as well as full filling visit requirements and communicating with families throughout the month.
Job Duties:
◻ Maintain your assigned case load and mandated monthly contact/visits with the families and follow up on service provision. We service all 5 boroughs your caseload may have cases that require you to do a visit in other boroughs. Complete monthly telephone log for record purpose.
◻ Must meet deadlines, no exceptions will be held responsible if not in compliance a written warning or suspension can be implemented for not being in compliance/audit ready. (Zero Tolerance)
◻ You will coordinate with DSP (Direct Support Professional) and individuals/families in order to assign a case to that particular family we service.
◻ Mandated to meet with DSP on a bi-weekly base in order to review individual summaries and to discuss individual progress reports.
◻ All monthly case notes and individual summaries must be filed by the 15th of each month (If the 15th falls on a weekend due date is last business day prior.
◻ Maintain data base with individual information and assigned DSP. Maintain database with required documentations due dates.
◻ All phones within the office must always be answered whether it is your direct line or co-workers.
◻ Must follow by dress code (Failure, employee will be sent home).
◻ eVero must be checked every morning first thing before start of day
Work with Individuals and direct support professionals.
Maintain up-to-date accurate documentation.
Minimum requirement: Associates
