Mercy Drive Inc. is seeking for a Outreach/Intake Coordinator.
SUMMARY OF RESPONSIBILITIES:
Intake: Assure prompt and accurate intake to the admission process including completed documentation, for all referrals.
Maintain current information on each prospect in the database and paper file.
Work collaboratively with all aspects of the clinical staff to ensure appropriate communication regarding prospects status and admission.
Outreach: Provide outreach in the community to educate and inform potential referral sources with knowledge of services through phone calls, scheduled presentations, and distribution of brochures.
JOB QUALIFICATIONS: • Extensive demonstrated experience in admissions coordination and community outreach.
- Ability to prioritize responsibilities and work independently as well as collaboratively.

SPECIFIC RESPONSIBILITIES: • Present the company and self in a positive and professional manner.
- Ensure professional relationship and team work when on site at each of the programs. •
Write and articulate clearly and effectively including plans, reports and all technical information.
- Act as liaison to community referral sources.
- Complete all intake to admission functions, including: Receiving referrals and informational phone calls, coordinating application process on prospective participants, gathering pertinent information from other providers (i.e. physicians, home care agency, social workers) scheduling and facilitating program tours, assessment meetings and scheduling admission days.
- Work closely with nursing to determine applicant eligibility and in conjunction with nurse make recommendation to Day Program team regarding appropriateness for admission.
- Work with Program staff, prospects, families and case managers to promptly resolve eligibility issues.
- Ensure all new participants have funding approvals prior to admission.
- Work closely with the Program managers and director on maintaining census at or above budgeted levels
- Develop and maintain an outreach referral list • Schedule and complete weekly outreach projects
- Develop a community education program to bring service providers into the programs on a regular basis for education about current healthcare interest topics and the values and benefits of services.
