This field-based role involves visiting patients to conduct physical and psychosocial assessments, documenting visits electronically, and developing nursing care plans. Visits occur in community and facility settings. The Hospice Field Case Manager makes care recommendations based on patient needs, works one weekend per month, and serves as a backup on-call 2-3 times monthly with car and escort service provided.
Qualifications include an RN license (BSN preferred), 2 years of experience in med/surg, community health, or oncology, and a valid driver's license.
