What are the responsibilities and job description for the Hospice Field RN Case Manager position at Aura Health?
This field-based role involves visiting patients to perform physical and psychosocial assessments, documenting visits electronically, and developing nursing care plans. Visits occur in community and facility settings. The position includes working one weekend per month and serving as a backup On Call 2-3 times monthly, with car and escort services provided.
Qualifications include an RN license (associates/diploma from an accredited school; BSN preferred), 2 years of med/surg, community health, or oncology experience, and a valid NYS driver’s license.
