Job Summary
We are seeking a compassionate and dedicated Case Manager to support individuals through complex social and behavioral health challenges. The ideal candidate will coordinate care, provide crisis intervention, and connect clients with essential resources. This role requires a strong understanding of case management principles to promote positive outcomes for clients in diverse settings.
Duties
- Conduct comprehensive assessments to identify clients’ needs related to behavioral health, overall welfare, and housing assistance.
- Develop individualized care plans and coordinate services across multiple agencies and providers.
- Provide crisis intervention and crisis management to ensure client safety and stability during emergencies.
- Support clients in navigating complex systems such as Medicaid, Medicare, Section 8 housing, and other community programs
- Maintain detailed documentation of case progress, service plans, and interventions in compliance with organizational policies.
- Collaborate with community partners to facilitate access to other supportive services.
- Advocate for clients’ needs within various systems to promote stability and independence.

Pay: $14.66 - $17.31 per hour
