This is a full-time, on-site Case Manager role based in Newton, NJ. The Case Manager will provide support and advocacy for individuals managing mental health challenges. Responsibilities include assessing client needs, creating individualized service plans, connecting clients to community resources, coordinating care, and monitoring progress to ensure successful outcomes. The Case Manager will also collaborate with multidisciplinary teams and actively engage with clients to provide emotional support and guidance.
- Strong assessment, planning, and case management skills
- Proficiency in building and maintaining relationships with clients and community resources
- Knowledge of local support services, mental health, and addiction recovery resources
- Excellent communication, organizational, and problem-solving skills
- Experience working in mental health or related advocacy/service fields is preferred
- Ability to work independently while collaborating with teams effectively
- Proficiency in documentation, record-keeping, and computer systems
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (or equivalent experience)
- Valid driver’s license and reliable transportation for local travel
