Job Overview
We are seeking a compassionate, organized, and motivated Case Manager to join our growing behavioral health and substance abuse treatment team. The Case Manager will work closely with clients, clinical staff, community providers, and families to help coordinate care, support treatment goals, and assist clients with accessing resources necessary for long-term recovery and stability.
The ideal candidate is professional, dependable, able to maintain strong boundaries, and passionate about helping individuals navigate recovery and behavioral health services. Duties
- Coordinate client care and assist with treatment planning alongside the clinical team
Knowledge, Skills & Abilities
- Conduct client needs assessments and assist with discharge and aftercare planning
- Connect clients with community resources including housing, employment, transportation, medical care, and support services
- Maintain accurate and timely documentation in the electronic medical record system
- Monitor client progress and communicate updates with the treatment team
- Assist clients with scheduling appointments and follow-up services
- Provide support, encouragement, and advocacy throughout the treatment process
- Maintain confidentiality and comply with HIPAA and company policies at all times
- Participate in team meetings, trainings, and supervision as required
- Assist with crisis intervention and client support when needed
- Build and maintain professional relationships with referral sources and outside agencies
- Other duties as assigned
Knowledge, Skills & Abilities
- Strong communication and interpersonal skills
- Ability to maintain professional boundaries in a behavioral health environment
- Excellent organizational and documentation skills
- Ability to work independently and as part of a multidisciplinary team
- Comfortable working in a fast-paced environment
- Knowledge of substance abuse and behavioral health treatment preferred
- Ability to handle sensitive information with professionalism and discretion
Qualifications
- Bachelor’s degree preferred in Social Work, Human Services, Psychology, Counseling, or related field
- Relevant experience in behavioral health, substance abuse treatment, case management, or healthcare preferred
- Experience working with individuals in recovery is a plus
- Valid driver’s license and acceptable driving record may be required
- Ability to pass background screening and drug screening
- CPRS Certified
Pay: From $50,000.00 per year
