JOB SUMMARY : The Mental Health Clinician (LMSW / LCSW) for the Crisis Stabilization Center plays a crucial role in providing comprehensive and evidence-based clinical support in the certified OMH/OASAS crisis facility.

The CSC is a 24/7/365 program for individuals in Mental Health/Substance Use Disorder (MH/SUD) crisis situations to access immediate, therapeutic support provided by professional and clinical health professionals; and for access to community resources to support their recovery upon discharge.

DUTIES AND RESPONSIBILITIES:

· Participate in the daily operations of the certified CSC supporting individuals with Mental Health/Substance Use Disorder (MH/SUD) challenges.

· Participate in all stages of client access to the CSC (intake, assessments, treatment plan development, referrals, discharge), working as part of a multi-disciplinary team.

· Complete Assessments: Complete thorough, strength-based, person-centered and trauma-informed psychosocial history with the client. Oversee/conduct clinical assessments.

· Develop an Individual Service Plan (ISP) with each client; ensure the collaboration of its use with the client and multi-disciplinary team (as applicable).

· Create a Safety Plan and/or Wellness Plan with each client; ensure the collaboration of its use with the client and multi-disciplinary team (as applicable).

· Care Coordinator Duties: Coordinate/refer client/family to community support services, including: mental health, substance use, peer support, medical care, housing, financial, legal, food, clothing. Provide information, resources and referrals related to the individual client’s need. Maintain ongoing communications with community partners; medical/social service professionals, criminal justice personnel and community organizations. Arrange transportation and/or teach client how to access community transportation supports.

· Provide ongoing observation in collaboration with the client and multi-disciplinary team (as applicable).

· Provide therapeutic interventions and ensure its use among the multi-disciplinary team; psychoeducation, crisis counseling, de-escalation, bridging and transition support, care collaboration, family support and engagement, health and wellness coaching, observation, outreach and engagement, peer support, planning for prevention of setbacks, overdose prevention, psychosocial assessment, skills development, wellness & safety planning, WRAP (Wellness Recovery Action Plan).

· Follow OMH and OASAS regulatory requirements for program operation.

· Follow MHAST policies and procedures.

· Ensure overall cleanliness and maintenance of the facility, ensure sufficient supplies and inventory for operations.

· Ensure a welcoming, calming, safe and secure environment in the center for clients and staff.

· Collaborate with other MHAST program staff to provide continuity of care to clients upon discharge.

· Collaborate with community partners and agencies that play a role in the SCSC (law enforcement, healthcare providers, mental health services, addiction services, transportation services, etc.).

· Attend MHAST staff meetings and trainings.

· Cooperate with audits, inspections, investigations.

· Flexibility to potentially cover shifts, in the event of call-ins or scheduling issues.


· Master’s degree in social work or related field; current NYS license required; LMSW/LCSW preferred. (Clinical supervision is provided by LCSW).

· Two or more years of experience working in MH/SUD field

· Two or more years of experience working with and/or treating children, youth, and adults

· Competence to engage and treat children and adolescents; at least 2 years of direct care experience working with children and families.

· Valid driver’s license and acceptable driving record to drive an agency vehicle.

· Successfully complete all agency required background and reference checks.

· Complete all state/agency required trainings during orientation period, and annually, as required.

· Maintain strict confidentiality and professional boundaries.

· Support the mission and vision of MHAST.

KNOWLEDGE/SKILLS/ABILITIES:

· Experience providing mental health services to individuals with a mental health condition.
· Knowledge of Substance Use Disorders (SUD) issues facing individuals in the community.

· Ability to handle crisis situations, traumatic events, or sensitive circumstances, support and advocate for individuals in crisis.

· Knowledge of community resources, services, and systems to assist individuals with Social Determinants Of Health (SDOH) needs.

· Understanding and sensitivity to support a diverse client population (race, religion, gender identity, sexual orientation, ethnicity, etc.).

· Trauma Informed, Person Centered.

· Experience with EMR/EHR systems.

· Knowledge of Medicaid/MCO billing.

· Knowledge of OMH and OASAS Regulations.

· Experience with Microsoft Office Suite (Excel, Word, Teams, etc.).

· Excellent organizational skills, detail-oriented.

· Excellent communication skills (verbal, written).
