- Conducting comprehensive patient assessments, developing and updating individualized care plans in collaboration with patients, families, and healthcare team members.
- Delivering direct patient care within the scope of practice, delegating tasks appropriately, and monitoring care quality.
- Ensuring patient safety by adhering to safety protocols and maintaining a safe environment.
- Utilizing performance metrics for quality improvement, evaluating care effectiveness, and implementing improvements.
- Teaching patients and families, assessing learning needs, and providing culturally sensitive education.
- Maintaining professional licensure, completing mandatory education, and adhering to regulatory standards (TJC, CMS, SC DPH).
- Documenting accurately and advocating for patients as a care coordinator.
- Associate degree in Nursing required; Bachelor's preferred.
- Minimum of one year RN experience or completion of a recognized nurse residency program or LPN experience (for internal candidates).
- Current RN license recognized by NCSBN Compact or in the practicing state.
- Strong clinical competency, communication, and collaboration skills.
