Key duties involve completing evaluations, discharge planning, and documenting patient care using Home Care Home Base EMR.
Must have a bachelor’s, master’s, or doctoral degree in Physical Therapy, an active Michigan license, and reliable transportation for travel within approximately one hour of the hub city.
Preferred experience includes previous home health work and comfort with video conferencing.
Work schedule is Monday through Friday, with no on-call duties.
