Home and Community Based Services Licensed Practical Nurse
Role: Provide direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions. Responsibilities include documenting patient visits, coordinating care activities, and ensuring high-quality care within legal and regulatory standards. Work under supervision of a Registered Nurse or designated supervisor.

Key Duties
- Deliver clinical services within the scope of practice, adhering to infection control protocols.
- Thoroughly document patient visits in electronic medical records and visit verification systems.
- Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members.
- Unrestricted LPN licensure and current CPR certification.
- Valid driver’s license, reliable transportation or access to public transit.
- Ability to work effectively in diverse home environments and independently.
- Preferred experience in home care, strong communication, organizational, and multitasking skills.
