What are the responsibilities and job description for the HHA and Transportation position at Mennonite Friendship Communities?
This position will be part-time in Home Health and part-time in Transportation. As an aide, you will assist residents with activities of daily living and meeting other needs as required, which promotes a high quality of life and a positive environment for independent living residents. In the Transportation role, you will be responsible for providing transportation and assistance to and from appointments for all residents campus wide.
Home Health Aide
Key Responsibilities
- Assist residents with dressing, grooming, and showering.
- Assist residents with housekeeping (including taking care of commode – if needed)
- Deliver meals to residents as requested.
- Assist IL Dietary Staff during the noon meal.
- Promote House & Home services in a positive manner.
- Assists residents with ADLs based upon their Home Health Plan.
- Assisting HH nurse with duties, e.g. picking up meds at pharmacy; faxing; scanning; attaching files in Matrix;ETC
- Emergency assistance
- Filing HH papers
- Transporting AL residents to hairdresser
- Toenail services
- BP Clinic
- Assisting residents with medication administration
- Documenting appropriately in the resident’s medical record and in MFC billing system.
- Other duties as assigned.
Communication
- Communicate with other home health staff
- Communicate in a professional and positive manner to all mfc staff, residents, and family members.
Transportation
Key Responsibilities
- Transport residents to and from doctor’s appointments, other appointments or other requests such as grocery shopping, airport transport, etc, which may include driving to surrounding cities and/or communities.
- Transports residents as requested to restaurants or dining facilities with permission from nurse and/or family.
- Assist resident into the doctor’s office or other appointment facility and accompany them as instructed.
- Contact supervisor once transport is complete.
- Deliver laboratory specimens and pick up prescriptions as requested by nursing department or supervisor.
- This will require being on call every other weekend.
Safety
- Ability to safely drive all mfc vehicles, including vans, cars, and buses. (One bus requires CDL license and cannot be driven unless driver possesses that type of license.)
- Maintain resident safety at all times within the vehicle and during transport.
- Ensure resident wheelchair is fastened appropriately in vehicle and seat belt is securely fastened.
Other Duties
- Maintain upkeep of mfc vehicles and report any maintenance issues to Transportation Coordinator.
- Participate in an “on-call” schedule for the evenings, weekends, and holidays for providing transporting on an “as needed” basis.
- Performs other duties as assigned.
Qualifications and Position Requirements:
- The desired minimum requirements necessary to effectively perform position responsibilities include a certificate as a Home Health Aide from an accredited educational provider; excellent leadership skills or an acceptable equivalent combination of education and experience, and respect for the principles of resident rights, confidentiality, EEO and ADA
- Must have a valid driver’s license and acceptable driving record.
Physical Demands:
- Requires frequent walking, standing, sitting, and bending, as well as frequently lifting up to 50 pounds.
