Our mission is to simplify and elevate men’s and women’s health.
We are a concierge health optimization and hormone replacement service, currently based in Arizona and recently expanded into Washington State. As part of this growth, we are seeking a dedicated and professional Provider licensed in Washington to help care for our expanding patient base in the state.
This role will play an important part in supporting our Washington clients, working alongside our experienced team as we continue to grow our presence in the region. We are looking for a provider that will manage care for both our Men's Revival and Women's Revival clients.

Role Description
- 1099 Independent Contractor
- Part-time to start (20 hour commitment), with goal to transition to full-time as patient load increases
- We are also open to long-term part-time providers

Qualifications
- Active, unencumbered Medical License valid in WA and DEA number
-At least 3 years experience
-Comfortable with virtual and in-person client visits
-Basic computer and EHR competency
-Self-motivated
-Strong attention to detail
-Team player
-Personable
-Clear and concise communication
-Passion for keeping up to date with the most current research and medical trends, especially as related to hormone replacement, peptides and overall wellness
Postion Responsibilities
-Ordering and interpreting labs
-Ability to perform basic physical examination
-Ability to review client’s medical history, symptoms and lab results to develop customized treatment plans and lifestyle recommendations

Work Location
This position will start remotely while we finalize the details for our new office in Bellevue, Washington. Our goal is to have the office operational by May, at which point the role will transition to a hybrid schedule combining in-office and remote work.
Initial training will take place at our Scottsdale, Arizona office.
