We are a homecare agency dedicated to providing compassionate, reliable care to clients in their homes. We are looking for a motivated Care Coordinator to manage client relations, scheduling, and support our caregivers.
Key Responsibilities:
- Conduct client intake and onboarding
- Develop and maintain care plans
- Schedule caregivers and coordinate visits
- Handle client inquiries, concerns, and complaints
- Follow up with clients/families to ensure satisfaction
- Maintain accurate documentation and records
- Support office operations, remotely where possible
Qualifications / Experience:
- CNA (Certified Nursing Assistant) certification is required
- CPR & First Aid certification is required
- Experience in healthcare, social work, or client services preferred
- Excellent organizational and communication skills
- Basic computer skills (scheduling software, email, Word/Excel)
- Compassionate, proactive, and reliable
- National Background check
