We are an outpatient pharmacy located on the south side of Chicago inside Jackson Park Hospital. We are seeking a dedicated and knowledgeable pharmacist to join our healthcare team in a Part-Time capacity (2 days/week). The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. As a pharmacist, you will be responsible for providing exceptional patient care, conducting thorough patient assessments, and collaborating with healthcare professionals to optimize medication therapy. This position requires a strong commitment to patient service and the ability to work in a fast-paced environment.
Duties
- Dispense medications accurately and efficiently while adhering to all legal and regulatory requirements.
- Conduct patient assessments to evaluate medication therapy and ensure optimal outcomes.
- Provide counseling to patients regarding medication usage, side effects, and interactions.
- Collaborate with healthcare providers in hospital settings, particularly in critical care and pediatrics, to develop comprehensive medication plans.
- Monitor patient progress and make recommendations for medication adjustments as necessary.
- Maintain accurate records of patient profiles, prescriptions, and medication administration.
- Educate patients and their families on proper medication administration and health management.
Skills
