Pharmacist

As a Full Time Long Term Care Pharmacist, you will be integral to providing high-quality pharmaceutical care. Reporting to the General Manager, your role will involve utilizing your clinical pharmacy experience and critical care knowledge to assess patient needs and manage medication therapies effectively. You will leverage your communication skills to collaborate with healthcare teams and ensure optimal patient outcomes. Your expertise in IV infusion and acute pain management will be essential in delivering comprehensive patient services, contributing to the overall health and well-being of residents.

Responsibilities

• Provide comprehensive pharmaceutical care to long-term care patients, ensuring optimal medication management and safety.
• Conduct patient assessments and collaborate with healthcare teams to develop individualized care plans.
• Monitor and evaluate patient responses to therapies, adjusting medications as necessary.
• Educate patients and caregivers on medication usage, potential side effects, and adherence strategies.
• Maintain accurate records and documentation in compliance with regulatory standards.
• Communicate effectively with healthcare professionals to enhance patient outcomes.

Requirements:

• Open availability
• Rotating weekends and Holidays
• Licensed Pharmacist in good standing in IL
• Ability to work well within a team structure: LTC pharmacists work as part of a team with pharmacy technicians, billing staff as well as other pharmacists
• Strong desire to provide the best service to our customer base
• Flexibility, ability to jump in where needed as needed throughout the work day
• Framework software experience and long-term care pharmacy experience are a plus
• Excellent communication skills

Pay: $56.00 - $62.00 per hour
