Location: Stamford, NY – Catskills Region
Schedule: Friday, Saturday & Sunday

Genesis Medical Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide weekend primary care coverage for geriatric patients within a skilled nursing facility in the Stamford, NY area .
This position is ideal for a provider looking for a consistent Friday through Sunday schedule with strong clinical independence and support from an established medical team.
Schedule
Friday, Saturday & Sunday
Daytime in-facility coverage
One overnight on-call shift required
Provider must remain accessible during scheduled coverage and on-call hours for urgent patient and facility needs
Responsibilities
Provide primary and acute care to skilled nursing facility residents
Evaluate residents for changes in condition and new medical concerns
Perform admissions, follow-up visits, and routine medical management
Review labs, imaging, and clinical reports
Coordinate care with nursing staff, physicians, specialists, families, and facility administration
Manage medically complex geriatric patients
Complete documentation and EMR charting in a timely manner
Respond to urgent clinical needs during assigned overnight call
Help reduce unnecessary hospital transfers through timely clinical intervention
Requirements
Active New York NP or PA license
Strong clinical judgment and ability to work independently
SNF, nursing home, primary care, or geriatric experience preferred
Comfortable managing medically complex patients
Comfortable taking overnight call
Reliable transportation required
