Physician (MD/DO) — Nursing Home Primary Care | Glens Falls & Albany Region, NY (Part-Time)
Location: Glens Falls & Albany Region, NY
Salary: $100,000 – $200,000 per year (depending on schedule and volume)
Schedule: Part-Time | 1–3 Days per Week | Flexible Hours | Availability between 8:00 AM – 6:00 PM
A medical group providing primary care services to nursing homes is seeking a Physician (MD/DO) to provide part-time coverage for skilled nursing facilities located in the Glens Falls and Albany region of New York.
This role focuses on delivering high-quality primary care to geriatric patients in a nursing home setting, with flexibility to build a schedule that fits 1–3 days per week.
Position Overview
The physician will be responsible for managing a panel of nursing home patients, addressing both routine and acute medical needs while ensuring high-quality, coordinated care in collaboration with facility staff.
Clinical Responsibilities
• Provide primary care services to geriatric nursing home residents
• Conduct routine and urgent patient visits
• Manage chronic conditions and acute medical issues
• Coordinate care with nursing staff and facility leadership
• Participate in CCM and RPM programs when applicable
• Document patient encounters in facility EMR systems (SigmaCare or PCC)
Requirements
• Board Certified or Board Eligible Physician (MD/DO)
• Active New York Medical License (or ability to obtain)
• Active Medicare and Medicaid provider ID
• Experience working with geriatric patients preferred
• Skilled Nursing Facility experience preferred
