Position Overview
The MDS Coordinator is responsible for ensuring the accurate and timely completion of MDS assessments in accordance with state and federal regulations. This individual will work closely with nursing, therapy, social services, and facility leadership to support quality resident care, maintain compliance, and maximize reimbursement opportunities.
Key Responsibilities

Complete and submit MDS assessments accurately and within required timeframes

Ensure compliance with all federal, state, and facility regulations related to the MDS process

Collaborate with interdisciplinary team members to gather clinical information and support comprehensive resident assessments

Participate in the development and implementation of individualized care plans

Monitor reimbursement opportunities and ensure appropriate documentation to support case mix and Medicare/Medicaid requirements

Conduct audits and review documentation for accuracy and compliance

Provide education and guidance to nursing and interdisciplinary staff regarding MDS requirements and documentation standards

Stay current on changes to MDS regulations, reimbursement methodologies, and industry best practices
Qualifications

Active New York Registered Nurse (RN) license required

Minimum of 1 year of experience as an MDS Coordinator in a skilled nursing facility or long-term care setting

Strong knowledge of the RAI process, MDS completion, care planning, and reimbursement systems

Thorough understanding of Medicare and Medicaid regulations

Experience working with SigmaCare preferred

Excellent organizational, communication, and leadership skills
