Key Responsibilities
- Provide comprehensive medical care toSNF patients, including assessments, diagnosis, and treatment planning
- Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions
- Ensureaccurateandtimelydocumentation in EMR
- Active Physician Assistant (PA)license in Wisconsin
- Active DEA license (or ability to obtain prior to start)-Required
- Minimum 1 year of clinical experience as an PA(SNF, geriatrics, internal medicine, or rehab experience strongly preferred)
- Experience managing adult/geriatric patient populations
- Strong clinical decision-making and time management skills
- Comfortable working independently in an SNF setting
- Skilled Nursing Facility (SNF) experience
- Background in internal medicine, geriatrics, or physical medicine & rehabilitation (PM&R)
- Experience with orthopedic, stroke, or pain management patients
- Monday through Friday
- Full-Time or Part-Time

Physical Requirements
- Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer.
- Must be able tolift upto 50 pounds.
- May be exposed to bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
- Must wear the required PPE per company and/or facility requirements.
- Mustcomply withfacility vaccination requirements.
